Chief Care Management Officer

Blackbird Recruiting seeks a seasoned leader to spearhead our care management initiatives. As Chief Care Management Officer, you will be responsible for developing and implementing care management strategies that foster a patient-centered approach to healthcare delivery.

Key Responsibilities:
  • Develop and implement care management policies and procedures to ensure alignment with NYS/OPWDD Health Home regulations.
  • Collaborate with the quality department to set quality goals and establish methods and tools for staff audits, evaluation, and assessments.
  • Spearhead the development and implementation of care management strategies, fostering a patient-centered approach to healthcare delivery.
  • Provide high-level oversight to the VPs of Care Management and Care Management workforce, ensuring their growth and success.
  • Collaborate closely with key stakeholders to identify opportunities for improvement and implement best practices in care management.
  • Monitor and evaluate the effectiveness of care management programs, identifying areas for growth and improvement.
Requirements:
  • Outstanding leadership qualities.
  • Proficient in understanding the coordination and management of services for individuals with intellectual or developmental disabilities.
  • Ability to negotiate multiple systems through interaction with other departments, including Not-For-Profit IDD providers and healthcare systems.
  • Solid verbal and written communication skills.
  • Ability to handle sensitive and confidential information.
  • Organizational skills.
  • Bachelor's degree in Human Services or a related field is required; Master's degree or licensed professional is preferred.
  • A minimum of 10 years' related Human Service experience, with 5 years in IDD; three years of previous experience at a senior management level.
