About the Role

We are seeking a skilled RN Infusion Liaison to join our team at Providence Home Services Oregon. As a key member of our infusion nursing team, you will provide direct nursing services related to infusion therapy for patients in various settings, including hospitals, homes, clinics, and alternate living sites.

Key Responsibilities
  • Provide education to patients discharging from area hospitals, focusing on infusion therapy and related care.
  • Collaborate with various providers, patients, caregivers, physicians, and payors to ensure seamless care coordination.
  • Deliver infusion nursing care in the home or clinic, working closely with patients and their families to achieve optimal outcomes.
  • Develop and maintain strong relationships with healthcare providers, payors, and community resources to ensure comprehensive care.
Requirements
  • Education to meet certification, license, or registration requirements.
  • Oregon Registered Nurse License upon hire, or Washington Registered Nurse License (Vendor Managed), or National Registered Nurse Compact License upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Unencumbered Registered Nurse License in state(s) of hire, and wherever care is delivered.
Preferred Qualifications
  • 1 year of experience with acute hospital care, infusion, or home health.
