Position Overview:
The Transition Coordinator is tasked with managing all aspects of the Community Connections Post-Graduate Program, ensuring effective operations and service delivery.

Key Responsibilities:
- Oversee and assess program personnel
- Collaborate with educational staff and administrators to identify students requiring transition assistance
- Design and execute personalized transition strategies, including employment, further education, community training, and travel support for individuals aged 18-22 with disabilities
- Provide training and orientation for educators, parents, and students on transition-related topics such as self-advocacy, benefits, and available adult services
- Facilitate connections to adult funding sources before students age out of services.

Qualifications:
Required:
- Licensure as an Administrator of Special Education in Massachusetts
- Transition Specialist Endorsement from Massachusetts DESE
- Advanced degree in special education administration or a related field
- At least five years of experience in transition services
- Strong communication skills, both verbal and written
- In-depth knowledge of special education laws and regulations
- Proven ability to supervise and mentor educational staff
- Excellent interpersonal skills for effective collaboration with various stakeholders.

Essential Duties:
- Supervise and evaluate all staff within the Community Connections Post-Graduate Program
- Promote staff development through ongoing training initiatives
- Assist program participants with daily instructional challenges and serve as a resource
- Monitor student progress and facilitate appropriate referrals to adult services
- Coordinate the transition of students from high school to the Community Connections Program
- Oversee the creation of individualized education plans for students with special needs
- Encourage the development of new programs to meet community needs.

Work Environment:
- The role involves working in both educational and community settings.

Physical Requirements:
- Must be able to travel locally and operate standard office equipment
- Ability to sit or stand for extended periods and lift objects up to 15 pounds.
