Job SummaryWe are seeking a highly skilled and compassionate Care Coordinator to join our team. In this role, you will be responsible for coordinating patient care, communicating with healthcare providers, and ensuring that our patients receive the necessary resources and support to achieve optimal health outcomes.
Key Responsibilities- Conduct thorough assessments of patients to develop individualized care plans, taking into account their medical, financial, and social needs.
- Collaborate with healthcare providers, patients, and families to implement care plans, monitor progress, and make adjustments as needed.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and address any concerns or issues that may arise.
- Identify and address potential barriers to care, such as lack of insurance or access to community resources, and work with patients and families to develop solutions.
- Maintain accurate and up-to-date records of patient care, including progress notes, treatment plans, and discharge instructions.
To be successful in this role, you will need:
- A graduate degree in nursing or a related field.
- At least 2 years of experience in patient care, preferably in a hospital or community-based setting.
- A valid RN license or temporary RN license from the Texas Board of Nursing, or a valid Compact RN license.
- Current BLS for Healthcare Providers certification from a recognized organization, such as the American Heart Association or American Red Cross.
