Referral and Care Coordinator

At Parkland Health and Hospital System (PHHS), we are committed to delivering exceptional patient care and experience. As a Referral and Care Coordinator, you will play a vital role in ensuring seamless access to our services.
Key Responsibilities:
1. Referral Management: Receive and review clinical referrals, ensuring they meet established criteria. Collaborate with clinical staff to determine the best course of action for each patient.
2. Scheduling and Coordination: Schedule appointments and coordinate care with internal and external physicians' offices, ensuring timely access to diagnostic and treatment services.
3. Patient Communication: Communicate effectively with patients, families, and healthcare providers to ensure a smooth transition of care.
4. Data Management: Utilize electronic medical records to document patient information, schedule appointments, and communicate with healthcare teams.
5. Liaison and Collaboration: Serve as a liaison between patients, families, physicians, and nurses to ensure coordinated care and optimal outcomes.
Requirements:
Education: High School Diploma or equivalent.
Experience: Two years of medical office or business office experience, including one year of referral and/or registration management experience.
Skills: Excellent verbal and written communication skills, with the ability to work effectively with diverse populations. Proficiency in MS Word and Excel software, as well as a working knowledge of PC operations.
Preferred Qualifications: Bilingual skills and a working knowledge of laws and regulations governing Medicare, Medicaid, and other funding sources.
