Job Title: Women's Health Care Coordinator

Job Summary:

The Women's Health Care Coordinator is a registered nurse responsible for coordinating patient-centered care and services for women Veterans. This role requires a strong understanding of women's health care and the ability to work collaboratively with patients, VA staff, and non-VA care providers to achieve seamless care.

Key Responsibilities:

  • Coordinate care for women Veterans, including mammogram coordination and follow-up.
  • Work with the Women Veterans Program Manager, Women's Health Medical Director, and Primary Care Leadership to ensure effective disease and chronic care management.
  • Provide support to the Women Veteran's Program by ensuring timely coordination of care and collecting, retrieving, and analyzing data as appropriate.
  • Function as a liaison and plan, coordinate, and perform a wide range of clinical, administrative, and technical functions to support quality case management of gender-specific services for veterans.
  • Work directly with patients, VA staff, non-VA care providers, and the Women Veteran Program Manager to achieve seamless care, facilitate patient satisfaction, reduce duplication of services or delay of care, and monitor quality of care and services received.

Requirements:

  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • Previous experience as a Women's Health Care Coordinator, Women's Veteran Program Manager, or Women's Health Maternity Care Coordinator is preferred.
