Licensed Practical Nurse Ambulatory Family Medicine Specialist

The Licensed Practical Nurse Ambulatory Family Medicine position at UVA Health Northern VA & Culpeper is a key member of the healthcare team, responsible for establishing a personalized relationship with patients and their support systems. The successful candidate will collaborate with other team members to provide care that promotes optimal health, utilizing best scientific evidence and compassion to assist patients during their health journey.
Responsibilities- Provides and documents patient care under the direction and supervision of the Assistant Nurse Manager/Nurse Manager or Clinic Administrator.
- Assesses, evaluates, and manages patients, participating in the collection of patient data as needed.
- Determines nursing care needs through recognition of relationships between data gathered and a patient's health status and treatment regimen.
- Identifies relevant resources based on necessity and availability.
- Communicates information to others responsible for or involved in the care of the patient, utilizing appropriate communication tools such as Situation, Background, Assessment, Recommendation, and Question (SBARQ) when communicating within the interdisciplinary team and when handing off patient care.
- Documents appropriate information in a timely manner.
- Participates as part of the interdisciplinary care team in planning individualized care, collaborating with the registered nurse to consider ongoing modification of care plan based on patient's needs, capabilities, and condition.
- Demonstrates core knowledge of biomedical and clinical sciences and the application of this knowledge to patient care, identifying nursing interventions and goals for the patient and reviewing goals with the registered nurse.
- High School Diploma required. Graduate of accredited Practical Nursing Program required. Completion of a nursing program as approved by the State Board of Nursing.
- 1 year of relevant experience required.
- Applicable state LPN license required.
- Ability to successfully complete general and department-specific skills validation and competency testing.
The job requires constant standing, frequently walking, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.
