The Licensed Practical Nurse serves as a vital member of the healthcare team at UVA Health Northern VA & Culpeper, fostering authentic relationships with patients and their support systems. Collaborating with other team members, this nurse provides care that promotes optimal health, utilizing the best scientific evidence and compassion to assist patients throughout their health journey.
Key Responsibilities- Provides and documents patient care under the direction and supervision of the Assistant Nurse Manager/Nurse Manager or Clinic Administrator.
- Participates in the collection of patient data as needed.
- Determines nursing care needs through recognition of relationships between data gathered and a patient's health status and treatment regimen.
- Identifies relevant resources based on necessity and availability.
- Communicates information to others responsible for or involved in the care of the patient.
- Participates in dialogue with the patient/chosen support system to identify the individualized care plan.
- Consistently utilizes appropriate communication tools such as Situation, Background, Assessment, Recommendation, and Question (SBARQ) when communicating within the interdisciplinary team and when handing off patient care.
- Documents appropriate information in a timely manner.
- High School Diploma required. Graduate of accredited Practical Nursing Program required. Completion of a nursing program as approved by the State Board of Nursing.
- 1 year of relevant experience required.
- Applicable state LPN license required.
The job requires constant standing, frequently walking, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.
