Licensed Practical Nurse: Ambulatory Care Specialist

We are seeking a skilled Licensed Practical Nurse to join our Ambulatory team as a Floater. As a key member of our healthcare team, you will provide high-quality patient care and support to our patients and their families.
Key Responsibilities- Provide assessment, evaluation, and management of patients in the ambulatory setting.
- Participate in the collection of patient data and determine nursing care needs through recognition of relationships between data gathered and a patient's health status and treatment regimen.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Collaborate with registered nurses to consider ongoing modification of care plans based on patient needs, capabilities, and condition.
- Document patient information in a timely and accurate manner.
- Current LPN licensure in applicable state required.
- BLS certification required.
- 1 year of experience in area of specialty preferred.
- Computer skills, phone call etiquette, and organizational skills required.
