Job Title: Clinical Care Manager

We are seeking a qualified Clinical Care Manager to fill the dual role of Homecare Case Manager and possible Home Care Aid Trainer. The ideal candidate will possess strong clinical skills, exceptional communication abilities, and a passion for educating and empowering caregivers.

Responsibilities:
  • Conduct comprehensive assessments of clients' medical needs, develop individualized care plans, and monitor their implementation.
  • Serve as the primary point of contact for clients, families, and caregivers, coordinating all aspects of care delivery and ensuring continuity of services.
  • Provide hands-on training and supervision to home care nurses and aides, including instruction on personal care tasks, medication administration, and safety protocols.
  • Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to optimize client outcomes and promote holistic care.
  • Maintain accurate and thorough documentation of client assessments, care plans, and interventions in compliance with regulatory standards and organizational policies.
Qualifications:
  • Current licensure as a Registered Nurse (RN) in the state of Washington.
  • Minimum of one year preferred, of clinical experience, with a preference for candidates with prior experience in homecare or case management.
  • Strong assessment and critical thinking skills, with the ability to develop and implement individualized care plans.
  • Excellent communication and interpersonal abilities, with a compassionate and client-centered approach to care.
  • Experience in training and mentoring caregivers preferred, with a commitment to promoting excellence in caregiving practices.
