Key Responsibilities:

Evaluate patient and support system requirements for resources based on:

  • medication oversight (psychiatry)
  • psychotherapeutic services (individual, group, partial hospitalization, intensive outpatient, residential)
  • continuous medical attention
  • housing and placement options (nursing facilities, assisted living)
  • financial assistance (applications for social security disability, HUD, food assistance, energy aid, etc.)
  • community support services (groups)
  • specific local resources (SAIL, CSP, group homes).
Ensure that discharge planning commences upon admission. Gather input from providers and support systems.
  • Assist in the creation and execution of transition, discharge, and aftercare strategies before the conclusion of behavioral health services.
  • Organize follow-up appointments for patients with primary care physicians, psychiatrists, and other therapy programs, as well as living arrangements.
  • Engage with referral sources and/or other treatment providers within the designated timeframe and document accordingly.
  • Coordinate community resources to facilitate discharge. Provide all necessary documentation to other providers to ensure continuity of care.
  • Solicit feedback from the treatment team and collateral partners to address the ongoing care needs of patients.
  • Collaborate with the treatment team and collateral partners to secure appropriate discharge resources.
  • Maintain an updated list of specific resources in alignment with best practices.
Deliver therapeutic services to patients.
  • Provide team coverage as permitted by state regulations, scope of practice, and competency under the guidance of the clinical operations supervisor.
  • Administer cognitive behavioral therapy, dialectical behavior therapy, behavioral activation, interpersonal psychotherapy, and other therapeutic services to patients in accordance with clinical standards and under supervision.
  • Implement goals, objectives, and interventions as outlined in the patient's individualized treatment plan.
  • Utilize knowledge and skills to gather and assess information regarding the patient's condition.
    • Identify specific individual needs.
    • Provide interventions tailored to the individual.
    • Facilitate individual skills development.
  • Practice trauma-informed care.
  • Engage in training under the direction of the clinical operations supervisor.
  • Consult with the treatment team and support system as necessary.
  • Participate in regular supervision sessions with the clinical operations supervisor in compliance with regulatory requirements.
  • Attend and contribute to interdisciplinary treatment team meetings.
Complete assignments and documentation within required timelines.
  • Obtain outcome assessments at admission, discharge, and throughout the stay as necessary.
  • Complete ongoing treatment documentation as mandated by program guidelines, including patient care notes, group documentation, and contact notes.
  • Support the acquisition of Releases of Information to ensure continuity of care and collaboration.
Foster a supportive and secure environment.
  • Ensure the safety of the area.
  • Provide a structured therapeutic environment where positive social, educational, and personal behaviors and coping skills are taught, reinforced, and enhanced.
  • Support the management of the therapeutic environment.
  • Respond to emergency situations as required.
Contribute to a collaborative team atmosphere by treating individuals with respect and integrity, utilizing direct communication and active listening skills.
  • Be receptive to change and actively support it.
  • Be open to diverse ideas and perspectives.
Additional Information:

Education and Training Requirements:
  • Bachelor's degree in human services or a related field is strongly preferred, or a minimum of five (5) years of relevant experience in case management or the mental health field.
  • State-specific substance use credential required or to be obtained within a specified timeframe.
  • At least two (2) years of experience working with individuals with mental health and substance use disorders.
  • Certification in CPR from the American Heart Association or American Red Cross is required within a specified timeframe. Annual re-certification is mandatory.
  • Formal training in managing aggressive patients is required within a specified timeframe. Annual re-certification is necessary.
