Care Manager RN Job Description

At Swedish Health Services, we are seeking a skilled and compassionate Care Manager RN to join our team. As a Care Manager RN, you will play a vital role in coordinating the care and services of selected patient populations through the acute care episode, across the continuum.

Key Responsibilities:

  • Coordinate the care and services of selected patient populations through the acute care episode, across the continuum.
  • Work collaboratively with inter-disciplinary teams, both internal and external to the organization, to improve patient care through effective utilization and monitoring of healthcare resources.
  • Assume a leadership role to achieve desired clinical, financial, and resource outcomes.
  • Communicate timely, relevant, and accurate information to all parties involved with a patient's care.
  • Develop and document a discharge plan through collaboration with the interdisciplinary team.
  • Ensure that all activities to facilitate and coordinate the plan are being implemented and that the plan is continuously modified based on the patient's changing needs.

Requirements:

  • Bachelor's Degree in Nursing or completion of an accredited Nursing program with a Bachelor's degree or higher in a healthcare field.
  • Washington Registered Nurse License.
  • 3 years Registered nursing experience in an acute care hospital.

Preferred Qualifications:

  • 1 year Hospital case management experience.
