Job Title: Travel Nurse RN Case Manager

Supplemental Health Care is seeking a skilled Travel Nurse RN Case Manager to join our team for a 13-week assignment at a partnering Long Term Acute Care facility in Seattle, Washington.

Job Summary:

We are looking for a dedicated and compassionate RN to provide high-quality patient care in a fast-paced LTAC environment. As a Travel Nurse RN Case Manager, you will be responsible for assessing patient needs, developing individualized care plans, and collaborating with the healthcare team to ensure optimal patient outcomes.

Responsibilities:
  • Assess patient needs and develop individualized care plans
  • Collaborate with the healthcare team to ensure optimal patient outcomes
  • Provide high-quality patient care in a fast-paced LTAC environment
  • Document patient progress and communicate with the healthcare team
  • Participate in quality improvement initiatives to enhance patient care
Requirements:
  • Active Washington RN License or Compact State Nursing License
  • 2 years of recent clinical nursing experience
  • Previous LTAC experience preferred
  • American Heart Association BLS
  • ACLS preferred
Benefits:
  • Competitive pay rate: $2,120 – $2,367 per week*
  • AM shift available
  • 13-week contract with possibility of extension
