Care Manager RN

Swedish Health Services is seeking a highly skilled Care Manager RN to join our team. As a Care Manager RN, you will play a critical role in coordinating patient care and services across the continuum.
Key Responsibilities:
- Coordinate patient care and services across the continuum, ensuring seamless transitions and optimal outcomes.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Communicate effectively with patients, families, and healthcare providers to ensure timely and accurate information exchange.
- Advocate for patients and families to access necessary resources and services.
- Monitor and evaluate patient outcomes, identifying opportunities for improvement and implementing changes as needed.
Requirements:
- Bachelor's degree in Nursing or related field.
- Current Washington State Registered Nurse license.
- 3 years of experience in acute care hospital setting.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
