Care Manager RN

Job Summary: We are seeking a skilled Care Manager RN to join our team at Providence Swedish. As a Care Manager RN, you will play a critical role in coordinating patient care and services across the continuum.
Key Responsibilities:
- Coordinate patient care and services across the continuum, working collaboratively with interdisciplinary teams to achieve optimal outcomes.
- Assess patients' biophysical, psychosocial, environmental, economic/financial, and discharge planning needs, and develop comprehensive plans to address these needs.
- Procure services and resources for identified patients and families, serving as an advocate to promote achievement of agreed-upon goals.
- Communicate timely, relevant, and accurate information to all parties involved with a patient's care, ensuring seamless transitions across the continuum.
- Develop and document discharge plans through collaboration with the interdisciplinary team, ensuring that all activities to facilitate and coordinate the plan are being implemented and continuously modified based on the patient's changing needs.
Requirements:
- Bachelor's Degree in Nursing or related field.
- Washington Registered Nurse License.
- 3 years Registered nursing experience in an acute care hospital.
- Preferred qualifications: 1 year Hospital case management experience.
