Job Title: CCS Adult Care Coordinator

We are seeking a highly skilled and compassionate CCS Adult Care Coordinator to join our dynamic team at Jewish Family Services, Inc. As a Care Coordinator, you will play a vital role in providing care coordination services to adults enrolled in the Comprehensive Community Services (CCS) program in the greater Milwaukee area.

About the Role:

The CCS Adult Care Coordinator will work closely with consumers, agency staff, and community professionals to identify recovery goals, develop customized recovery plans, and provide support to improve or maintain the consumer's quality of life, independence, health, and overall wellness.

Responsibilities:
  • Meet with consumers in their homes or out in the community to assess their needs and develop recovery plans.
  • Work with consumers and a dedicated team of providers to decide which programs and supports will assist the individual in reaching their recovery goals.
  • Provide case management services, including monitoring and supporting consumers with behavioral health, mental health, and/or addiction issues.
  • Collaborate with community professionals to ensure a comprehensive approach to care coordination.
  • Document all interactions and progress in the Electronic Health Record (EHR) management system.
Requirements:
  • Bachelor's degree in a Human Services field (e.g. Social Welfare, Psychology, etc.)
  • Minimum of one year of paid formal experience working with adults providing case management or related services
  • Experience in behavioral health, mental health, and/or addiction is preferred
  • Excellent interpersonal, organizational, critical thinking, problem-solving, creative, and independent thinking skills
  • Ability to work independently with minimal supervision
  • Excellent verbal and written communication skills
  • Ability to maintain confidentiality and respect consumer privacy
