Job Title: CCS Adult Care Coordinator

We are seeking a highly skilled and compassionate CCS Adult Care Coordinator to join our dynamic team at Jewish Family Services Inc. in Milwaukee. As a key member of our Care Coordination team, you will play a vital role in providing rehabilitative and recovery services to individuals struggling with mental health or substance abuse issues.

Job Summary:

The CCS Adult Care Coordinator will work closely with consumers, agency staff, and community professionals to identify recovery goals, develop customized recovery plans, and provide ongoing support to improve or maintain the quality of life, independence, health, and overall wellness of our consumers.

Responsibilities:

  • Conduct assessments, monitor, and support CCS consumers with a professional and caring methodology.
  • Meet with individuals in their homes or out in the community to identify recovery goals and develop customized recovery plans.
  • Work with consumers and a dedicated team of providers to decide which programs and supports will assist the individual in reaching their recovery goals.
  • Provide ongoing support and case management services to consumers.
  • Collaborate with agency staff and community professionals to ensure comprehensive care and support.

Requirements:

  • Four-year Bachelor's degree in a Human Services field (e.g. Social Welfare, Psychology, etc.) is required.
  • A minimum of one (1) year of paid formal (non-family setting) experience working with adults providing Case Management or related services is required.
  • Experience in behavioral health, mental health and/or addiction is preferred.
  • Excellent interpersonal, organizational, critical thinking, problem solving, creative and independent thinking skills are required.
  • Ability to work independently with minimal supervision and prioritize tasks based on severity of need.
  • Excellent verbal and written communication skills are required.
  • Ability to maintain consumer confidentiality and respect their dignity at all times.
  • Familiarity with Electronic Health Record (EHR) management systems is required.
  • Proficient in computer use, Microsoft Office applications.
