Job Title: CCS Youth Care Coordinator

We are seeking a highly skilled and compassionate CCS Youth Care Coordinator to join our dynamic team at JFS, Inc. The successful candidate will provide care coordination services to members enrolled in the Comprehensive Community Services (CCS) program in the greater Milwaukee area.

About the Role

The CCS Youth Care Coordinator will work with individuals to identify recovery goals, develop customized recovery plans, and provide support to improve or maintain their quality of life, independence, health, and overall wellness. This is a full-time, predominantly first shift position requiring a valid Wisconsin Driver's License, access to a vehicle, and personal motor vehicle insurance meeting the minimum requirements of the Agency.

Responsibilities
  • Provide care coordination services to CCS members, including assessments, monitoring, and support.
  • Develop and implement individualized recovery plans in collaboration with consumers, agency staff, and other community professionals.
  • Meet with consumers in their homes or out in the community to identify recovery goals and develop customized recovery plans.
  • Work with a dedicated team of providers to decide which programs and supports will assist the individual in reaching their recovery goals.
  • Maintain accurate and timely case notes, submit reports, and meet deadlines.
  • Collaborate with other agencies and community resources to provide comprehensive support to CCS members.
Requirements
  • Four-year Bachelor's degree in a Human Services field (e.g. Social Welfare, Psychology, etc.) is required.
  • A minimum of one (1) year of paid formal (non-family setting) experience working with adults providing Case Management or related services is required.
  • Experience in behavioral health, mental health and/or addiction is preferred.
  • Excellent interpersonal, organizational, critical thinking, problem solving, creative and independent thinking skills are required.
  • Ability to work independently with minimal supervision and prioritize tasks based on severity of need.
  • Excellent verbal and written communication skills are required.
  • Ability to maintain consumer confidentiality and respect their privacy and dignity at all times.
