Job Summary

Commonwealth Care Alliance is seeking a skilled Nurse Assessment Specialist to join our team. As a key member of our Assessment unit, you will conduct initial and ongoing face-to-face assessments for our enrollees, ensuring timely and accurate submissions to MassHealth. This role requires a registered nurse with a strong clinical background, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Conduct in-person assessments and clinical exams, capturing members' clinical needs and meeting regulatory requirements.
  • Medication reconciliation, adherence concerns, vital signs, and acute and chronic disease education.
  • Complete additional tools or screeners as needed, including relevant behavioral health screeners.
  • Utilize electronic health records and care management platforms to collect data, document member interactions, and communicate with the care team.
  • Ensure immediate referral to time-sensitive services, such as behavioral health, long-term services, and support.
  • Manage assessment activities through the calendar and track the status of completion.
  • Participate in assessing and improving quality within the scope of responsibilities and throughout the organization.

Requirements
  • Associate degree in Nursing
  • Active registered nursing license in the Commonwealth of Massachusetts, in good standing.
  • 1+ years of clinical experience, including clinical rotation.
