Position Title: Case Manager

Reporting To: Program Manager

Work Schedule: Monday to Friday, 8:00 AM - 4:30 PM

Employment Status: Non-Exempt (Hourly), Full-Time, Non-Management

Overview

  • Assist clients in achieving housing stability, self-sufficiency, and community integration.
  • Work collaboratively with clients to identify and overcome barriers to housing placement.
  • Implement strategies to support housing retention and coordinate various services.
  • Educate and model essential life skills for a successful transition into permanent housing.
Key Responsibilities:

  • Manage a caseload of clients with high needs for case management services.
  • Build relationships that emphasize independence and enhanced well-being.
  • Collaborate with mental health and health service programs.
  • Engage clients using a harm-reduction approach.
  • Support the screening, intake, and enrollment processes for clients.
  • Conduct assessments and develop treatment plans.
Qualifications:

  • Bachelor's degree preferred or equivalent professional experience.
  • Valid driver's license and access to reliable transportation.
  • Familiarity with case management practices is preferred.
  • Comfortable working with clients facing diverse challenges.
Work Environment:

  • Combination of fieldwork and office settings.
  • Regularly required to sit, stand, bend; may occasionally lift or carry up to 35 lbs.
  • Must be able to work in dynamic environments.
  • May involve bending, stooping, twisting, and sitting throughout the day.
