End-of-Life Care Assistant

Under the guidance of the Clinical Manager or designated supervisor, the Hospice Aide is responsible for delivering essential patient care services aimed at addressing the personal needs and comfort of individuals facing terminal illnesses across various environments.
The Hospice Aide collaborates with the interdisciplinary team to ensure high-quality patient care. Weekend and holiday shifts are a requirement.
Key Responsibilities:
- Provides personal assistance, including light housekeeping and meal preparation, as outlined in the care plan.
- Exhibits knowledge of standard safety protocols and performs basic infection control measures.
- Educates family members or caregivers on effective techniques to fulfill the personal care requirements of the patient.
- Conducts personal care tasks to enhance and sustain patient hygiene.
- Assists patients with daily living activities.
- Supports patients in mobility and exercise routines.
- Aids patients with medications that they typically administer themselves.
- Utilizes proper body mechanics to ensure patient safety.
- Reports any changes in the patient's condition or care needs that necessitate follow-up to the supervisory team or appropriate personnel, encompassing physical, spiritual, social, and emotional aspects.
- Documents care provided in accordance with agency standards.
- Maintains patient confidentiality at all times.
- Participates in team meetings and other agency gatherings as required.
- Completes a minimum of twelve (12) hours of training/education relevant to the position annually.
- Performs additional duties as assigned.
All personnel in this role must be adaptable and willing to work across various Community Hospice locations.
Qualifications:
- Education/Experience: High school diploma or GED equivalent preferred.
- Minimum of six (6) months of combined experience in home care, hospital, and long-term care settings. Experience with terminally ill patients is preferred.
- Current licensure as a Certified Nursing Assistant (CNA) in the state of Florida.
- Possession of a valid Florida Driver's license with a driving record acceptable to the Community Hospice insurance provider; reliable transportation is necessary.
