The Case Manager - Transitional Housing Specialist will be responsible for providing case management services to clients in transitional housing. This role will involve working with clients to develop and implement individualized plans to achieve their housing and employment goals.
Key Responsibilities:
- Conduct initial assessments of clients to identify their needs and develop service plans.
- Provide referrals for emergency services, such as food and shelter, as needed.
- Assist clients with transitioning to permanent housing and provide support with job readiness and employment services.
- Develop and maintain relationships with community resources to provide clients with access to necessary services.

Requirements:
