Job Summary

We are seeking a skilled Case Manager to join our team at One East Harlem, a residential building providing supportive housing to homeless or at-risk individuals with serious mental illness or co-occurring disorders. As a Case Manager, you will work closely with tenants to develop and implement individualized plans, connect them with community resources, and advocate for their needs.

Key Responsibilities
  • Conduct thorough assessments and evaluations of tenants to identify their strengths and challenges
  • Develop and implement individualized plans to help tenants achieve their goals
  • Connect tenants with community resources, services, and support groups
  • Provide ongoing support and guidance to tenants to help them navigate challenges and achieve self-sufficiency
  • Monitor and document tenant progress, making adjustments to plans as needed
Requirements
  • Bachelor's degree in a related field (e.g., social work, psychology, counseling)
  • Experience working with individuals with serious mental illness or co-occurring disorders
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
What We Offer

CAMBA offers a competitive salary range of $40,000-$45,000 annually, as well as a comprehensive benefits package, including health insurance, dental insurance, and a 403(b) retirement plan with employer match.
