Creative Arts Therapist/Case Manager

Housing Works is seeking a highly skilled and compassionate Creative Arts Therapist/Case Manager to join our team. As a key member of our AIDS Adult Day Health Care (AADHC) program, you will provide case management services, supportive counseling, and therapeutic services to clients living with HIV/AIDS.
Responsibilities- Manage a caseload of active clients, providing case management services, including monthly assessments, quarterly assessments, quarterly care planning, counseling, outreach, case conferences, advocacy, referrals, and discharge planning.
- Develop and deliver diverse and meaningful clinical creative arts group interventions.
- Establish treatment goals and objectives with clients as part of the interdisciplinary team.
- Facilitate up to 6 groups per week, attend all trainings, staff meetings, care plan sessions, and daily morning rounds.
- Provide crisis intervention services and refer clients for psychiatric assessments as needed.
- Coordinate the provision of multidisciplinary services as outlined in interdisciplinary care plans.
- Identify clients receiving services from other community providers and assume the role of lead case manager.
- Develop a collaborative relationship with clients to monitor integration in the AADHC and encourage client attendance.
- Present psycho-social needs, strengths, and limitations of assigned clients during interdisciplinary care planning meetings and case conferences.
- Participate in continuing quality assurance and quality improvement committee activities to ensure the highest level of services to all enrolled clients.
- A master's degree in creative arts therapy with a limited permit and moving to be licensed in NYS to practice Creative Arts Therapy (LCAT).
- At least two years of experience in providing case management services to HIV/AIDS-infected substance-using populations.
- Skills in group facilitation and counseling are preferred.
- The ideal candidate should be able to employ a variety of clinical techniques, theories, and skills to elicit sensitive information, handle crises, and work effectively in an interdisciplinary team.
- Strong organizational skills and proficiency in Spanish are highly preferred.
