Work Schedule:
Full-time, 40 hours per week with flexible evening shifts and rotating weekends.
Position Overview:
The Therapist is responsible for the development, coordination, and delivery of direct social services to patients and their families. This role involves working within an acute care environment that encompasses partial hospitalization and intensive outpatient services.
Key Responsibilities:
- Deliver therapeutic treatment services to individuals, groups, families, and significant others.
- Conduct individual therapy sessions based on program requirements and patient needs.
- Provide crisis intervention services and conduct daily safety assessments.
- Engage with patients and their families with respect and dignity.
- Facilitate psychoeducational support sessions.
- Manage casework and therapeutic interventions for all assigned patients.
- Act as an advocate for patients in grievance situations, adhering to system policies.
- Maintain effective communication with referring providers and the care team.
- Implement and document goals and interventions as outlined in the Care Plan.
- Ensure that goals are SMART (specific, measurable, attainable, relevant, and timely).
- Document patient progress in accordance with treatment plans.
Documentation Responsibilities:
- Complete all required admissions documentation.
- Conduct Social Services Admission Assessments and Suicide Risk Assessments.
- Develop safety plans as necessary.
- Document ongoing treatment progress and maintain clear records.
- Summarize patient progress during multidisciplinary team meetings.
Leadership and Communication:
- Evaluate interdisciplinary interventions and adjust Care Plans as needed.
- Participate in treatment planning and progress reviews.
- Communicate effectively with treatment team members and other departments.
- Demonstrate professionalism in all interactions and maintain a positive team environment.
Professional Conduct:
- Exhibit strong organizational skills and timely responses to patient needs.
- Promote a collaborative atmosphere among all disciplines.
- Adhere to ethical standards and maintain confidentiality.
Patient Safety:
- Ensure patient safety and report any concerns promptly.
- Follow emergency protocols and document safety incidents.
Qualifications:
- Master's Degree in Social Work or a related field.
- In-training licensure as a Professional Counselor, Marriage and Family Therapist, or equivalent.
- Knowledge of therapeutic interventions for various age groups.
- Completion of required training in aggression management and patient safety.
