About the Role:

The Housing Retention Case Manager will provide comprehensive support to low-income individuals and families to help them maintain stable housing and achieve long-term self-sufficiency.

Key Responsibilities:

  • Conduct thorough assessments of household strengths and needs to develop personalized goals and strategies for housing retention
  • Provide case management services to families in crisis, including crisis intervention and advocacy
  • Collaborate with community partners to ensure access to all necessary resources and services
  • Develop and maintain relationships with clients, families, and community stakeholders to promote housing stability and family well-being
  • Document all interactions and progress in client files, maintaining confidentiality and adhering to agency policies
  • Participate in regular team meetings and training sessions to stay up-to-date on best practices and agency initiatives

Requirements:

  • Bachelor's degree in social work, human services, or a related field (Master's degree preferred)
  • Two years of experience providing related services to families and individuals (one year with a Master's degree)
  • Knowledge of community-based services, public assistance, and other community resources
  • Demonstrated skills in accessing and referring clients to these services
  • Excellent written and verbal communication skills, with the ability to represent the agency in a professional manner
  • Ability to work independently and as part of a team, with a strong commitment to client-centered services
  • Valid New York State driver's license, regular access to a car, and willingness to travel to off-site locations
