About the Role

We are seeking a highly skilled RN Clinical Liaison to join our team at Gentiva, a leading provider of hospice, palliative, and personal care services. As a RN Clinical Liaison, you will play a critical role in educating healthcare professionals about our services and providing end-of-life solutions that prioritize clinical excellence, compassion, and dignity.

Key Responsibilities
  • Ensure effective and timely intake and referral processes to meet the needs of patients, families, and referral sources
  • Complete admissions processes for emergency cases and inpatient hospice admissions
  • Provide accurate and timely information about hospice services to inquiring individuals
  • Respond to calls from referral sources, initiate paperwork, and begin the intake/referral process
  • Serve as a patient advocate in evaluating and developing plans of care and obtaining informed consent for hospice
  • Call on multiple account types, including SNFs, ALFs, hospitals, and physicians, to offer in-services and education on the benefits of hospice
Requirements
  • RN license in the state(s) of employment
  • Minimum of 2 years of healthcare experience in hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator
  • Known and respected within your community
