Key Responsibilities
  • Evaluate patient progress and revise care plans as needed.
  • Deliver safe and effective basic care to assigned patients while developing technical competencies.
  • Coordinate clinic visits to optimize patient care.
  • Communicate patient needs and recommend treatment to care team members.
  • Provide direct nursing care and document patient information in electronic health records.
  • Implement and coordinate specific caseload strategic interventions to move patients through the continuum of care.
Requirements
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • One year of specialized nursing experience equivalent to the next lower grade level.
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN) degree.
  • Master's degree in nursing (MSN) or a related field.
  • Experience in telehealth and technology.
