Key Responsibilities:
  • Coordinate patient services through an interdisciplinary approach, ensuring a clinical and psychosocial focus throughout the care continuum.
  • Assess patients to determine the appropriateness of admission, continued hospitalization, and the optimal level of care.
  • Facilitate timely care delivery in the right setting, adhering to CMS guidelines and collaborating with stakeholders.
  • Develop and implement discharge plans in partnership with the clinical team, patients, and their representatives.
  • Monitor and report quality and risk management issues as necessary.
Requirements:
  • Associate's Degree (or higher) in Nursing
  • Current, unrestricted RN license in the state of employment
  • 1+ years of experience with community resources, policies, and procedures
  • 1+ years of clinical experience with age groups, including the unborn child through geriatric age groups
Preferred Qualifications:
  • Bachelor of Science
  • 2+ years of case management experience
  • Case Management Certification
  • Strong analytical, critical thinking, and organizational skills
