Position Overview

Client Support Coordinator - Safe Haven IHH (Full Time)

Safe Haven provides a comprehensive range of supportive services tailored for individuals experiencing homelessness and actively engaging with substance use, frequently accompanied by concurrent mental health challenges. The Integrated Health Home (IHH) team comprises a diverse group of professionals, including psychiatrists, nurses, case managers, peer recovery specialists, hospital liaisons, and housing advocates.

The primary objective is to facilitate care coordination and address the social determinants of health, aiming to enhance overall health and well-being. The client support coordinator plays a crucial role in this mission, collaborating closely with clients to eliminate obstacles to care.

Support services include:

  • Linkage to emergency shelters and housing solutions, ensuring access to essential needs,
  • Coordination of primary healthcare services,
  • Assistance with benefits and entitlements, including legal navigation,
  • Development of improved natural support systems and social networks.

The methodology employed is centered around the individual and is informed by trauma awareness. This role is not only impactful but transformative. Each day presents unique challenges. Strong verbal and written communication skills are essential, along with the ability to maintain professional boundaries with clients. Proficiency with technology and familiarity with Electronic Health Records are required.
