The Community Support Program (CSP) is seeking a dedicated and empathetic individual for the role of Intake Specialist. This full-time position plays a crucial role in crafting detailed health service plans, overseeing daily organizational tasks, and monitoring client engagement with health coordination and preventive initiatives.
Key Responsibilities:
- Lead the development of comprehensive health service plans.
- Coordinate daily organizational and treatment/service planning activities.
- Monitor client responses to health coordination and prevention strategies.
- Assist in orienting new clients and their families to CSP and health home services.
Ideal Candidate Profile:
- Exhibit proficiency in clinical, technical, or administrative skills.
- Collaborate effectively within a multi-disciplinary team in a dynamic environment.
- Transport clients and conduct home visits as necessary.
- Demonstrate initiative and manage multiple tasks independently.
Education and Licensing Requirements:
- Bachelor's degree preferred in psychology, social work, or a related human services field.
- An Associate's degree or equivalent education in the human services sector is required.
- Preferred one year of experience in community mental health.
- Completion of CCSP training is necessary.
- Must possess a reliable vehicle, auto insurance, and a valid driver's license.
