Position Overview

The Community Support Program (CSP) is seeking a dedicated and empathetic individual for the role of Intake Specialist. This full-time position is pivotal in crafting detailed health service plans and overseeing the daily organization and treatment/service planning for our clients.

Compensation:
$30.23 per hour

Key Responsibilities:
- Lead the development of comprehensive health service plans.
- Monitor client responses to health coordination and preventive measures.
- Provide case management services to adults with psychiatric disabilities and their families, ensuring they maintain and enhance their community functioning.
- Assist in orienting new clients and their families to CSP and health home services.

Ideal Candidate Profile:
The successful candidate will:
- Exhibit proficiency in clinical, technical, or administrative competencies.
- Collaborate effectively within a multi-disciplinary team in a dynamic environment.
- Transport clients and conduct home visits as necessary.
- Demonstrate initiative and manage multiple tasks efficiently with minimal supervision.

Education and Experience Requirements:
- A Bachelor’s degree in psychology, social work, or a related human services field is preferred.
- An Associate’s degree or equivalent in the human services area is required.
- A minimum of one year of experience in community mental health is preferred.
- Completion of CCSP training is essential.
- A reliable vehicle, valid driver’s license, and auto insurance are mandatory.
