We are looking for an experienced Clinical Care Specialist to join our team in Concord, California. As a member of our team, you will have the opportunity to work closely with patients and their families, providing essential social work services.
About the PositionThis 13-week contract position offers a unique opportunity to work in a dynamic environment and make a meaningful impact on patient care.
Responsibilities- Develop and implement care plans that address the emotional, social, and environmental needs of patients and their families.
- Collaborate with healthcare teams to coordinate services and resources for patients.
- Document all interactions and progress in patient records in compliance with regulatory standards.
- Current California BBS Certification is required.
- Basic Life Support (BLS) certification from AHA only is required.
- Minimum of 2 years of experience as a social worker in a healthcare setting.
- Active California licensure is required; acceptable licenses include LCSWs, ACSWs (also known as MSWs), LMFTs, and AMFTs (also known as MFTs).
- Strong communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team in a dynamic environment.
The estimated salary for this position is $55-$65 per hour based on location and industry standards.
