About the Role:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Centre Avenue Health & Rehab. As an LPN, you will play a vital role in the recovery and well-being of our post-acute and elderly patients, providing personalized care and support to ensure their physical, emotional, and psychological needs are met.

Responsibilities:
  • Administer prescribed medications and treatments accurately, observing resident response
  • Collect samples, such as urine, blood, and sputum for residents
  • Assist nursing assistants in attending to the individual needs of residents
  • Monitor, observe, report, and record physical, mental, and psychological changes
  • Perform delegated independent nursing functions according to each resident's plan of care using established procedures, policies, guidelines, and standards
Requirements:
  • Nursing degree from an accredited college or university
  • Clear unencumbered license from the State of Colorado as a Licensed Practical Nurse
  • Current CPR Certification
