Job Title: LPN - Centre Avenue Health & Rehab

Columbine Health Systems is seeking a skilled Licensed Practical Nurse (LPN) to join our team at Centre Avenue Health & Rehab. As a valued member of our healthcare team, you will play a critical role in the recovery and well-being of our post-acute and elderly patients and residents.

Responsibilities:
  • Provide high-quality care to residents in a manner that enhances their quality of life
  • Administer prescribed medications and treatments accurately, observing resident response
  • Collect samples, such as urine, blood, and sputum for residents
  • Assist nursing assistants in attending to the individual needs of residents
  • Monitor, observe, report, and record physical, mental, and psychological changes
Requirements:
  • Nursing degree from an accredited college or university
  • Clear unencumbered license from the State of Colorado as a Licensed Practical Nurse
  • Current CPR Certification
  • New Grad LPN's Welcome as well as any licensed nurse within a hospital, long-term care or any other related healthcare facility
Benefits:
  • Competitive starting wages $40.00/hr. base pay based on years of experience, plus shift differentials
  • Evening differential $3.00
  • Overnight differential $4.00
  • Weekend shift differential $3.00
