About the Role

Columbine Health Systems is seeking a skilled Licensed Practical Nurse (LPN) to join our team at Lemay Avenue Health & Rehab in Fort Collins. As a valued member of our care team, you will play a vital role in providing high-quality care to our post-acute and elderly patients and residents.

Responsibilities
  • Provide direct patient care, including administering medications, collecting samples, and monitoring vital signs.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
  • Nursing degree from an accredited college or university.
  • Clear unencumbered license from the State of Colorado as a Licensed Practical Nurse.
  • Current CPR Certification.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
