Job Summary

The Office of Housing and Employment's Housing Integration Unit has an immediate opening for a Coordinated Personal Care (CPC) Program Manager.

This project role is responsible for ongoing development, statewide expansion, and improvements to the innovative personal care service provision model of CPC for Medicaid Transformation Project (MTP) and Long-Term Service and Supports (LTSS).

The successful candidate will work closely with regional Home and Community Services (HCS) and Area Agency on Aging (AAA) staff, Long-Term Service and Supports (LTSS) providers, community partners, and department staff to create successful transitions and stabilize in independent housing for the clients we serve.

Key Responsibilities
  • Develop and update statewide policies, procedures, for the CPC model of service delivery to ensure field operations and service providers meet rules and requirements of funders.
  • Participate in and/or facilitate stakeholder and partnership meetings for the development and implementation of expansion plan for the CPC service delivery model for MTP and LTSS.
  • Point of contact for field staff statewide for all CPC policy clarifications and interpretations, including the dissemination of information to the field.
  • Draft concept papers, utilizing data and feedback, for executive management on proposed legislation to support the expansion of the CPC service provision model and/or promote paid home care workforce expansion ideas learned from the work on the expansion of the CPC model.
  • Develop and provide training, technical assistance, and coordination in collaboration with MTP and Medicaid program managers inclusive of AAA and HCS field, service provider, and AAA contract managers as needed.
Requirements
  • Professionals with a bachelor's degree in health or social science, public administration, health information systems, or related field AND five years' experience planning and/or administrating community-based supportive services for seniors and/or people with disabilities.
  • Additional qualifying professional experience will substitute year-for-year for the required education.
  • Experience in and knowledge of principles and practices of home and community-based service delivery systems, payment systems, vendor contracts, and client eligibility rules for functional and financial eligibility requirements.
  • Must have the ability to communicate and comprehend, through oral and written communication with leadership, peers, clients, professional staff, legislators, and the public.
  • Ability to produce accurate work products that are timely with minimal supervision.
