Job Description:

The Case Manager III performs highly complex case management work, overseeing case management activities for all persons at the RGSC Outpatient Clinic. This role involves developing and maintaining long-term contact with patients and families, as well as service providers, to ensure medical, social, and educational needs are met. The Case Manager III is accountable for developing, facilitating, and partnering with community resources to address patient needs. This includes coordinating medical referrals, interpreter services, and other services through intra-agency or interagency service coordination and referral incidents.

Key Responsibilities:
  • Develops and maintains long-term contact with patients and families
  • Coordinates medical referrals and interpreter services
  • Partners with community resources to address patient needs
  • Ensures continuity of care when patients are transferred between agencies
  • Prepares and submits monthly, quarterly, and annual reports or performance improvement plans
Requirements:
  • Graduation from an accredited four-year college or university with major coursework in social work or a related field
  • At least 2 years of experience in patient case management
  • Able to communicate in both English and Spanish
Working Conditions:

This role requires flexibility in work hours, with the possibility of overtime and extended hours. Compliance with HHSC immunization policy and state hospital operating procedures related to immunizations is required.
