Job OverviewFirst Match Services, Inc. is seeking a highly skilled Inpatient Care Coordinator to join our team. As a key member of our healthcare team, you will serve as a communication link between patients and healthcare professionals, ensuring seamless care delivery and optimal patient outcomes.

Key Responsibilities

  • Identify and coordinate post-acute care settings to ensure patients receive the best possible care
  • Collaborate with patients and their families to provide support and information throughout the care process
  • Work closely with healthcare professionals to ensure efficient and effective care delivery


Requirements

  • Registered Clinician with RN, PT, or OT credentials preferred
  • Current active unrestricted clinical license required
  • 3-5 years of clinical experience required, with at least 1 year of recent skilled nursing facility experience
  • Experience working with geriatric population preferred
  • Bachelor's degree preferred
  • Case Management experience with CCM preferred
