Substance Use Counselor

VNS Health is seeking a skilled Substance Use Counselor to join our team in Queens, New York. As a key member of our Behavioral Health team, you will play a vital role in providing compassionate care and support to individuals struggling with substance use and mental health challenges.
Key Responsibilities:
- Conduct phone and field outreach to locate and enroll clients in programs and services
- Build relationships and trust with clients and their families, assisting with interactions with professionals on the team
- Work collaboratively with an interdisciplinary team to discuss care needs and identify solutions to support clients
- Apply shared experiences to build relationships and trust with clients
- Educate clients about program services, benefits, and self-help techniques
- Advocate for effective recovery-based services on behalf of clients
- Teach and model symptom management and coping skills for resilience
- Review service plans with clients and their families, providing ongoing education, guidance, support, and encouragement
- Develop inventory of resources to meet client needs
- Provide navigation services to help clients connect with community-based services and supports
- Document in EMR in accordance with program policies, VNS Health standards, and regulatory requirements
Requirements:
- High school diploma or equivalent required
- Minimum of one year experience in a mental health, substance use treatment program, healthcare, or human services setting (preferred)
- Experience working with severely mentally ill, psychological trauma, and/or substance using populations (preferred)
- Effective oral, written, and interpersonal communication and relationship building skills required
- Ability to work independently and collaboratively on an interdisciplinary team
- Computer literacy (electronic health records, word processing, email, internet research, data entry) required
- Valid New York State driver's license (as determined by operational/regional needs)
- Bilingual skills in English and Spanish (preferred)
