Job Summary:

Ohel Children's Home and Family Services is seeking a skilled Licensed Mental Health Clinician to join our innovative team. This role will involve facilitating the transition of participants from higher levels of care to the community, providing counseling, skill development, and connecting them with community providers.

Key Responsibilities:

  • Engage in transition planning and needs assessment to ensure a smooth transition for participants.
  • Monitor discharge and ongoing care planning to ensure participants receive the necessary support.
  • Build collaborative relationships with community providers to ensure participants receive comprehensive care.
  • Provide short-term counseling and risk assessment to identify potential issues.
  • Resolve clinical issues affecting housing stability and provide crisis intervention and support services as needed.
  • Assist with appointment navigation and referrals to ensure participants receive the necessary care.
  • Conduct comprehensive psychosocial assessments to identify participants' needs and develop individualized plans.
  • Monitor participant progress and goal achievement to ensure participants are meeting their goals.
  • Conduct family conferences and psychoeducation to educate participants and their families on mental health topics.

Qualifications:

  • Masters degree in Social Work or Mental Health Counseling or a related field required (LMSW, LCSW, LMHC).
  • Knowledge of mental illness, substance use disorders, and NYC behavioral health programs.
  • Knowledge of techniques for identifying and preventing potentially violent behavior, including crisis management techniques.
  • Ability to develop, evaluate, implement, and modify a clinical treatment intervention to meet the needs of individual recipients.
