Job Summary:

MercyFirst is seeking a skilled Case Manager to support and stabilize families involved with Child Welfare or in crisis. The successful candidate will utilize the Solution Based Casework model to prevent out of home placement and provide case work counseling to address issues concerning Intimate Partner Violence, Mental Health concerns, Substance Use, and School difficulties.

Key Responsibilities:

  • Provide case management services to families in crisis
  • Utilize the Solution Based Casework model to develop and implement individualized plans
  • Collaborate with community resources to provide support and services
  • Conduct home visits to assess the safety of children
  • Document and maintain accurate records of case management activities

Requirements:

  • Bachelor's degree in a related field (Social Work, Psychology, Mental Health)
  • 1+ years of related work experience with children and/or families preferred but not required
  • Proficiency in Microsoft Word and Outlook
  • Excellent communication skills (verbal and written)
