Job Summary:The Advanced Practice Provider II will work closely with our medical staff to provide comprehensive care to patients in designated primary or specialty areas. This includes obtaining histories, performing physical exams, making assessments, ordering tests, and determining diagnoses. You will also be responsible for developing and implementing individualized care plans, educating patients and families, and collaborating with other healthcare professionals to ensure optimal patient outcomes.
Responsibilities:- Provides comprehensive patient care, including obtaining histories, performing physical exams, making assessments, ordering tests, and determining diagnoses.
- Develops and implements individualized care plans, taking into account patient preferences, values, and goals.
- Educates patients and families on health promotion, disease prevention, and management of chronic conditions.
- Collaborates with other healthcare professionals to ensure optimal patient outcomes, including physicians, nurses, and other advanced practice providers.
- Participates in quality improvement initiatives to enhance patient care and safety.
- Stays current with the latest developments, advancements, and trends in healthcare by attending seminars, workshops, and professional conferences.
- Master's degree in nursing or a related field, or a Doctor of Nursing Practice degree.
- Current licensure as an Advanced Practice Registered Nurse (APRN) with authorization to practice in the state of Texas.
- Active certification as a Nurse Practitioner by a recognized certifying organization.
- At least 3 years of experience as an Advanced Practice Provider in a healthcare setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
