About the Role

We are seeking a highly skilled Care Coordinator to join our team at Innovista Medical Center in Houston, Texas.

Job Summary

The Care Coordinator will work under the direction of the Care Manager, handling health service coordination and clerical tasks, allowing licensed professionals to focus on complex clinical interventions.

  • Conduct initial and pre-visit patient outreach, including health risk screenings, assessments and patient-centered care plans.
  • Assist in scheduling appointments, arranging transportation, and ensuring patient compliance.
  • Support Care Managers and LCSWs with care coordination prioritizing patients for case management as needed. Also, conducting follow-ups based on patient needs.
Responsibilities
  • Provide evidence-based educational promotion from the center's content library and assist in patient education regarding preventive care and services.
  • Facilitate transitions of care by coordinating with the patients, caregivers, medical providers, and community resources.
  • Collaborate with the interdisciplinary care team (ICT) to ensure effective communication and coordination of care across various providers and community organizations.
Requirements
  • High school diploma or equivalent required.
  • 2-3 years of experience as a Care Coordinator in a health care setting.
  • Bilingual in English and Spanish required.
