Job Title: Transplant Coordinator

At Yale New Haven Health, we are seeking a highly skilled and compassionate Transplant Coordinator to join our team. As a key member of our transplant program, you will play a vital role in ensuring the seamless transition of patients from pre-transplant to post-transplant care.

Responsibilities:
  • Coordinate the evaluation process for patients referred for cardiac transplantation, utilizing fiscal responsibility to minimize patient and hospital costs.
  • Develop and maintain clinical files of patients evaluated and/or listed for cardiac transplantation, interfacing with insurance carriers to ensure a seamless transition from pre-op to post-transplant phase.
  • Participate in interdisciplinary medical transplant rounds to discuss the current status and plan of care for all patients currently in-house on the medical service.
  • Collaborate with nurse managers, primary nurses, and medical staff to plan, implement, and evaluate changes in practice and healthcare delivery on inpatient and outpatient units.
  • Coordinate the transplantation process when a donor is available, communicating with physicians, local organ procurement organizations, patients, and families.
  • Communicate effectively with multidisciplinary team members, patients, and families in a timely manner, resolving issues relating to transplant patients.
  • Assess post-transplant organ function and general health status, determining the need for additional assessment or intervention.
  • Assist nurses and other staff in assessing, planning, implementing, and evaluating care for transplant patients.
  • Collaborate with care coordinators and other team members to coordinate discharge plans for patients post-transplant.
  • Participate in the development and implementation of protocols and standards of care for transplant patients in collaboration with the multidisciplinary team.
  • Educate patients along the continuum to assist in understanding processes and procedures surrounding pre- and post-transplant issues.
  • Prepare recipients for discharge via intensive interval teaching.
  • Refer patients or families for assistance utilizing health system and community resources as appropriate.
  • Complete necessary documentation according to UNOS guidelines, including listing, status change, and post-transplant follow-up information.
Qualifications:

Bachelor's degree in Nursing required. Two or more years of clinical nursing experience in intensive care, cardiac step-down, or organ transplantation highly desirable. Excellent basic nursing and medical knowledge, clinical skills, and judgment required. Analytical skills to evaluate care needs, ability to gather relevant clinical data, and excellent interpersonal skills, communication, and leadership activities essential.

Candidate should have 3-5 years of cardiovascular experience, strong communication and teamwork skills, BLS certification, and experience with EPIC and Microsoft Office preferred.
