Job Summary

We are seeking a highly organized and detail-oriented Cardiac Therapy Coordinator to join our team at Yale-New Haven Health. As a key member of our transplant team, you will play a critical role in coordinating the scheduling and testing of patients referred for advanced cardiac therapies.

Responsibilities
  • Coordinate evaluations for patients referred for advanced cardiac therapies, ensuring timely and accurate data entry.
  • Maintain the transplant waiting list, identifying and resolving any issues that may impact patient care.
  • Interface with hospital departments and personnel to ensure seamless communication and documentation.
  • Work closely with the Histocompatibility Lab to ensure accurate blood typing and listing of patients on the national registry.
Qualifications

We are looking for an individual with an Associate's Degree in Allied Healthcare or a related field, and at least two years of related clinical experience. Excellent organizational and communication skills are essential, as well as the ability to handle multiple tasks and interact with all levels of staff, patients, and families.

Additional Requirements

Familiarity with Epic Phoenix, SiteCore, and Scheduling software is a plus, as is previous experience as a transplant associate coordinator. If you are a team player with excellent time management skills and a passion for delivering exceptional patient care, we encourage you to apply.
