Job Summary

We are seeking a highly skilled and dedicated Clinical Transplant Coordinator RN to join our team at Ochsner Health. As a key member of our transplant team, you will play a vital role in ensuring the smooth transition of patients through the transplant process.

Key Responsibilities
  • Perform pre-transplant and post-transplant clinical activities, including patient education and coordination of care.
  • Collaborate with transplant physicians and other healthcare professionals to ensure optimal patient outcomes.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Participate in quality improvement initiatives to enhance patient care and safety.
  • Provide education and support to patients and their families throughout the transplant process.
Requirements
  • Current State of Louisiana RN License
  • 3 years related experience with RN Diploma or RN Associate Degree or two years related experience with a BS Degree in Nursing
  • Preferred: Certified Clinical Transplant Coordinator (CCTC) through the American Board of Transplant Certification (ABTC)
