Position Overview

This role involves conducting onsite visits to clients' residences, educational institutions, and program locations to foster relationships and assess service requirements and support systems.

Key Responsibilities
  • Develop, review, and monitor the Person-Centered Service Plan (PCSP) in compliance with regulatory standards.
  • Document individual progress and concerns, ensuring timely completion of referrals to community organizations.
  • Facilitate meetings and program visits to empower member and family decision-making.
  • Complete referrals for evaluated and covered services.
  • Collaborate with community organizations to provide assistance and support to members and families experiencing crises.
  • Compose detailed progress notes and maintain comprehensive case records.
  • Prepare various reports and undertake special projects as necessary.
  • Participate in staffing meetings, training sessions, and other relevant gatherings.
  • Confer with supervisors and assume supervisory responsibilities in their absence.
Qualifications

The ideal candidate will possess strong communication skills, a commitment to client advocacy, and the ability to work collaboratively with various stakeholders to enhance service delivery.
