Addiction Counselor II

As an Addiction Counselor II at ADDICTION AND MENTAL, you will play a vital role in providing comprehensive care to individuals struggling with addiction. Your expertise will help patients navigate the recovery process, and your dedication will make a meaningful difference in their lives.
Key Responsibilities:
Conduct comprehensive psychosocial assessments to identify patient needs and develop personalized treatment plans.
Evaluate and integrate assessment findings from all disciplines and databases into a clinical summary.
Develop and implement treatment plans with specific goals, objectives, and corresponding interventions.
Clearly and legibly document patient progress and treatment outcomes in progress notes.
Manage and coordinate patient care, ensuring timely and effective treatment delivery.
Formulate and document discharge summaries that summarize patient progress and treatment outcomes.
Requirements:
Master's degree in Social or Behavioral Science required.
Professional licensure is required.
Minimum of two years working experience in addictions treatment.
Previous training and/or experience demonstrating competency with a special population (e.g., Adult or Adolescent population).
Ability to administer CPR and be certified.
Emotional and physical capability to function under stressful conditions.
Maintain records and possess good communication skills.
Working Conditions:
Must be able to walk approximately 1/2 mile per 8-hour shift.
Must be able to sit for 1/2 hour to 4 hours per 8-hour shift.
Must be able to see (corrected) and hear (hearing aid acceptable).
If recovering, three years of continuous verifiable sobriety.
