Job Summary

We are seeking a skilled LPN to join our Primary Care team at Kaiser Permanente. As a key member of our healthcare team, you will provide high-quality patient care and support to our clinicians and other healthcare professionals.

Key Responsibilities
  • Provide clinical support to clinicians and other healthcare team members, including preparing patients for office visits, performing focused nursing assessments, and implementing planned nursing and medical interventions.
  • Contribute to the development, implementation, and modification of individualized patient plans of care under the direction of a clinician.
  • Utilize standardized tools to identify and address health maintenance and care gaps with patients and alert clinicians to those that require attention.
  • Perform diagnostic tests, therapies, and procedures within the scope of practice, competently and with maintenance of proficiency.
  • Assist in coordinating patient care to support patients with complex health needs in navigating the healthcare system.
Requirements
  • Graduate of an accredited school of nursing.
  • Practical Nurse License (Washington) within 6 months of hire or Compact License: Practical Nurse within 6 months of hire.
  • Basic Life Support certification.
  • Demonstrated clinical nursing skills, interpersonal and customer service skills, and communication skills.
Preferred Qualifications
  • Two (2+) years of nursing experience in an ambulatory care setting.
  • IV therapy experience or recent certification.
  • Familiarity with documenting in an electronic medical record system.
